It shows how non-physicians may gradually take on physician-like roles under ambiguous institutional frameworks.
Layer 1: Support Role
Dentists are allowed to assist physicians in drafting pathology reports—but only under supervision.
Layer 2: Functional Imitation
Some dentists begin to influence or guide the diagnostic process—effectively acting as diagnosticians, despite lacking accountability.
Layer 3: Social Assimilation
By mimicking credentials (e.g., writing “MD” in publications), they come to resemble true physicians.
At this point, the figure begins to resemble what we later define as a shadow MD—
a non-physician who appears to be a doctor through explicit credential misrepresentation and prolonged influence.
